The easiest option is to complete the required forms during your child’s Annual Wellness Check. A sports physical is included as part of that visit. We provide:
Important Billing Information
Most insurance plans fully cover an Annual Wellness Check at no cost to you. Many plans also offer rewards or premium discounts for completing one.
If your child is seen outside of their annual wellness visit for a sports physical only, there is a $50 flat fee, as this type of visit is not typically covered by insurance.
Scheduling the sports physical during the annual visit is often the most comprehensive and cost-effective option.
When your child wakes up sick, you shouldn’t have to wait days to be seen. We offer same-day appointments and extended evening and weekend hours to care for urgent but non-emergency concerns.
